SUBCONTRACTORS

This form is for subcontractors providing a service. Examples include, but are not limited to:

Architects, engineers, designers, flooring, masons, framers, concrete, painters, etc.

We require the following from all of our subcontractors. These are required to set you up for work and before any payments are made. A minimal subcontractor package will be emailed if we end up working together.

- Insurances:

General Liability is required for everyone

Commercial auto is required if you're delivering materials

You will be required to list us as additionally insured and as certificate holders.

- Independent Contractor Agreement: spells out the terms of our relationship. Protects us both.

- W9: we submit 1099's and require a tax ID

- DWC85: workman's comp signoff

- ACH info: we do not pay by cash, check, zelle, etc. We direct deposit.

This form is for suppliers wishing to sell us product. We only buy wholesale; either direct to the supplier or through their authorized distributor.
